The state of Illinois law requires all students in kindergarten, second, and sixth grade have a dental examination by May 15 of the school year. The dental exam can be dated up to 18 months prior to May 15, 2022. Below is the link for the dental examination form that needs to be completed by your dentist and returned to the nurse. If you have already turned in your form, please disregard.
If have not already done so, please email Nurse Johnson ([email protected]) or drop off a copy of your child’s vaccination card if they are fully vaccinated. Current IDPH guidance states that vaccinated students do not have to be quarantined if they are in close contact with a student that tests positive for COVID-19.

With suburban Cook County now in a level of high community transmission of COVID-19 (Cook County Department of Public Health), District 54 has decided to create COVID Dashboards on each school’s website to provide families and staff with a transparent summary of instances of COVID-19 reported to District 54. A positive case indicates that the student or staff member has tested positive for COVID-19. Please report cases of COVID-19 to our school so that we can initiate contact tracing. Parents/guardians of students identified as close contacts will be contacted directly to discuss next steps. District 54 will update these dashboards by 3 p.m. on Fridays when school is in session.
